This form is for those wishing to include the names of their loved ones for inclusion in this year’s Memorial Prayer Booklet wish accompanies the Yizkor service.
We kindly ask that details be submitted no later than Thursday 20 August to allow sufficient time for the booklet to be finalised before printing. We regret that we will be unable to include any submissions received after this date.
